The type of personal information we collect
We currently collect and process the following information, which will be kept for 7 years (or until a child reaches a minimum of 25 years old)
· Personal identifiers, contacts and characteristics (for example, name, address and contact details)
. Doctors details. Some medical history, if required.
. Next of kin details. In case of emergency during a session.
. Some brief personal notes may be taken during a session, this will be kept to a minimum, stored anonymously and securely.
How we get the personal information and why we have it
Most of the personal information we process is provided to us directly by you on the intake form, for one of the following reasons:
. To have a clear understanding about you, what support you need, what support you have already had and how I may be able to help you further.
. Next of kin details are for in case of emergency during the session. The only time private information would be shared with them is if I believe you may harm yourself or others. This would be discussed with you first.
. Doctors information would only be used if I believed you may harm yourself or others.
Under the General Data Protection Regulation (GDPR), the lawful bases we rely on for processing this information are:

How we store your personal information
Your information is securely stored digitally and secured with a password.
We keep all the details mentioned above for 7 years, or up to aged 25 for a child. We will then dispose of your information by deleting the files and shredding any paper notes, if any.

Under data protection law, you have rights including:
Your right of access - You have the right to ask us for copies of your personal information.
Your right to rectification - You have the right to ask us to rectify personal information you think is inaccurate. You also have the right to ask us to complete information you think is incomplete.
Your right to erasure - You have the right to ask us to erase your personal information in certain circumstances.
Your right to restriction of processing - You have the right to ask us to restrict the processing of your personal information in certain circumstances.
Your right to object to processing - You have the right to object to the processing of your personal information in certain circumstances.
Your right to data portability - You have the right to ask that we transfer the personal information you gave us to another organisation, or to you, in certain circumstances.
You are not required to pay any charge for exercising your rights. If you make a request, we have one month to respond to you.

Refunds Policy: Refunds can be given for sessions booked individually if 48 hours’ notice is given. Any unused Pre-Paid blocks will not be refunded. There are 60 days in which to use the block-booking sessions.
How to complain
If you have any concerns about our use of your personal information, you can make a complaint to me at kellyshadbolt@gmail.com . I will rectify if I am able to, and take it further if necessary.
You can also complain to the ICO if you are unhappy with how we have used your data.
